The template

18 questions in six groups.

Enough to know who a family really is; short enough that good families actually finish it.

1 · Buyer basics

  • Full name, email, phone, and preferred contact method.
  • City, state, and whether they are willing to travel.
  • How soon they are hoping to bring a puppy home.

Why it matters: The timing question quietly separates dreamers from buyers. A family hoping to bring a puppy home 'someday' belongs on your long list; a family ready in eight weeks belongs in your review pile.

2 · Home and lifestyle

  • Who lives in the home, including children and other pets?
  • What does a normal weekday look like for the family?
  • Where will the puppy sleep, train, and spend most of the day?

Why it matters: The weekday question predicts fit better than square footage ever will. A family that can describe a normal Tuesday has already pictured the puppy in it.

3 · Puppy preferences

  • Preferred sex, size, coat, temperament, and energy level.
  • Pet, therapy, sport, guardian, breeding, or service prospect interest.
  • Any allergies, grooming expectations, or deal-breakers.

Why it matters: Deal-breakers surfaced now prevent go-home-week fallout. It is far easier to steer a family before they fall in love with the wrong puppy.

4 · Experience and readiness

  • Past dog or puppy experience.
  • Training plan, schedule, and support system.
  • Vet relationship and readiness for puppy costs.

Why it matters: The vet question is the honesty check. Families with a real plan name a clinic; families without one say 'we'll figure it out.'

5 · Litter fit and timing

  • Which current or planned litter they are interested in.
  • Whether timing matters for travel, work, school, or holidays.
  • Whether they want to stay on the waitlist for a future litter.

Why it matters: Flexibility here is your matching leverage. A family open to a future litter gives you options when the current one doesn't line up.

6 · Agreements and expectations

  • Confirmation they understand deposits, contracts, and pickup timing.
  • Agreement to answer honestly and keep contact details current.
  • Anything else the breeder should know before reviewing the application.

Why it matters: Putting deposits and contracts in the application makes the money conversation normal instead of awkward. Nobody is surprised later.

Why not just a Google Form?

A form gives you responses. This gives you a waitlist.

A generic form leaves you sorting a spreadsheet of disconnected answers. The same questions inside Expert Breeder become organized waitlist records — connected to your dogs and litters, free forever.
